Services

For patients who qualify for services, we provide Nurse Practitioner visits for in home primary care and palliative care (symptom management) for those who find it difficult or cannot get to other providers and need home visits on a regular basis or as needed for symptom management.

CARE ASSOCIATES

Southwestern Palliative Care Associates is an association of providers that consist of services as needed by Nurse Practitioners, Registered Nurses, Certified Emergency Paramedic, LNA, Social Worker, Chaplain, Physician Oversight when needed.

Once you have chosen SWPCA to provide care in your home, we will make a visit to assess your needs. We can become your Primary Care Provider (PCP), order your medications, tests and facilitate needed resources for you.

End of Life Care

Once your condition has declined and you have been deemed to have 6 months or less in your life expectancy, we can continue to love you until the end.  However, you do have a choice and, other Hospices are available.
SWPCA will refer you based upon your choice.

If you chose SWPCA & Hospice, your Hospice Team Includes customized services by:
Registered Nurses, 
Licensed Nurse Aides/Home Health Aides, Social Workers, Trained Volunteers, 
Counselors/Chaplain's, Medical Director, and Ancillary Therapists

Providers will work with the Hospice Team to design and oversee a plan of care and to coordinate treatment, medical equipment, supplies and medications.

Prayer

The desire of the entire SWPCA team is to be the hands and feet of Jesus. We believe at SWPCA that we serve the Lord by serving others. We don't want to be just another Hospice, but to be a light in the community of Yuma Arizona.

BEREAVEMENT

Grieving is often an overwhelming and painful period that everyone will experience differently. It is an inevitable part of life that we all must deal with. SWPCA offers a dedicated bereavement program for the families that are left behind after the loss of their loved one, who was in our service.
